If that was the case everybody would just pay $5 a month regardless of what they owed and no business would be able to stay open. Every bank would fail. Sorry, that's not how it works. Hospitals have staff they have to pay, supplies they have to order, and it requires a steady income stream to happen. If they can't count on a certain revenue, they can't pay their vendors and their credit gets shut down. If you're not contributing your part, they have every right to send you to collections. You’re not allowed to just pay what you feel like if it’s not what the hospital has agreed to.
